I've been trying to get this problem resolved in order to run the latest cm12 build or fml with the dirty_v kernel but haven't arrived to a solution. Basically for whatever reason my phone can't handle booting at 512mhz gpu (which is the default clock on the latest cm12) so I need to find a way to lower the gpu clock to 384 before boot. Is there a script that could do this for me? and yes i've tried both twrp and cwm along with doing several full wipes and trying different versions of gapps as to isolate the issue to the gpu.
